- You have had the opportunity to ask questions and all your questions have been answered to your satisfaction.

- You agree to provide accurate and complete medical history, including medications, allergies, and past procedures, to assist in your safe treatment.

- You accept financial responsibility for all costs associated with the procedure that are not covered by Medicare or your insurer.

- You agree to notify the clinic at least 24 hours in advance if you need to cancel or reschedule. Late cancellations may incur a fee.

- You consent to the collection and storage of your health information in accordance with our Privacy Policy.

- While all care is taken, you acknowledge that no treatment can be guaranteed to be risk-free or to achieve a particular outcome.

These terms are governed by the laws of Victoria, Australia.
